Replacement Licence Cards & Certificates

If you have lost your licence card or training certificate, you can request a replacement from the NTDA.

To process your request, please email info@ntda.co.uk with the following information:

• Your full name

• Date of birth

• The training provider you completed the course with

• The company you were working for at the time of the training

• Whether you require a replacement licence card or replacement certificate

• A recent photo of yourself so we can confirm your identity

Fees

• Replacement licence card: £15 + VAT

• Replacement certificate: £5 + VAT

Once we have received your request and verified your details, we will contact you to arrange payment and issue your replacement.
